- Tamariki (please complete for those under 18. Those older will need to complete their own registration form)
- Whakapapa- On your father’s side
- Whakapapa- On your mother’s side
- Hapuu and Marae
- Acknowledgement
PLEASE READ/NOTE:
As mentioned above; All who descend from Ngāti Apakura that are 18 years of age or older, will need to complete their own registration form and include their tamariki under them. On successful completion, you will receive an email to acknowledge your application.
